Descending septal artery: an important artery for collateral coronary circulation.

چکیده
We present a case report of a 40-year-old male who underwent elective cardiac catheterization secondary to complaints of intermittent chest pain and inducible ischemia in the anterior wall. Diagnostic catheterization revealed severe coronary artery disease including an occluded mid left anterior descending (LAD) artery. There was a rarely described descending septal artery (Bonapace's branch) originating from a separate ostium in the right aortic sinus. This artery was a very important source of collateralization to the LAD. This artery has been reported once in vivo, while it has been described more frequently in postmortem studies. The descending septal artery is postulated to have an important role in the perfusion of the conduction system and in collateral circulation in patients with coronary artery disease. A literature review confirms the functional importance of this artery.

Ischemia in the Territory of the First Major Septal Perforator Branch Anomalously Originating From the First Diagonal Branch Leads to a Transient Leftward Shift of the QRS Axis in the Frontal Plane
rigination of the first major septal perforator branch from a site other than the left anterior descending coronary artery (LAD) is a relatively rare and benign congenital variation. 1 Coronary angiography reveals anomalous vessels originating directly from the aorta and from the proximal portion of the right coronary artery as collateral circulation to the LAD, particularly in patients with pr...
متن کاملLate appearance of traumatic ventricular septal defect.
A ventricular septal defect with delayed appearance is describedfollowing stab wound of the heart in a woman. The diagnosis was confirmed by left ventricular angiography. Damage to the left anterior descending coronary artery, documented by selective coronary arteriography, might have played an important role in late development of the shunt. The septal defect was closed successfully by a patch...
متن کاملCollateral circulation from the conus coronary artery to the anterior descending coronary artery: assessment using multislice coronary computed tomography.
The prognosis of patients with coronary artery disease largely depends on the presence of a collateral circulation. The location and extent of the collateral circulation is highly variable and these parameters determine whether or not ischemic symptoms occur and whether left ventricular contractility is abnormal. The collateral circulation is generally established through small-caliber distal v...
متن کاملAssociation Between Left Anterior Descending Artery Length with Coronary Artery Dominance: An Angiographic Study
Introduction: Although left anterior descending (LAD) artery supplies a large portion of the myocardium, the amount of blood supply provided by LAD is depended on the length of the LAD. The aim of the current study was to evaluate the association between coronary artery dominance and LAD anatomic types in patients with normal epicardial coronary arteries. Methods:...
متن کاملCovered stent septal ablation for hypertrophic obstruction cardiomyopathy.
Obstruction Cardiomyopathy To the Editor: Fifer et al1 report a case of hypertrophic obstructive cardiomyopathy in which a covered stent was implanted in the left anterior descending artery to occlude the first septal branch after an unsuccessful attempt to cannulate it. Recurrence of symptoms was associated with collateralization from the right coronary artery of the occluded septal branch. We...
